Personal Care

Discreet, respectful support that protects dignity and privacy. We follow your preferences and any care plans in place.

  • Showering, bathing, oral hygiene, hair and skin care
  • Dressing, undressing and choosing outfits
  • Toileting and continence support in line with care plans
  • Safe manual handling and transfers with appropriate equipment

Behavioural Support & Positive Strategies

We implement positive behaviour supports focused on safety, communication and choice. Where a Behaviour Support Plan exists, we work to the plan and collaborate with your behaviour support practitioner.

  • Understanding triggers and using de-escalation and coping strategies
  • Consistent routines, visual supports and communication aids
  • Data and incident notes to inform practitioner reviews
  • Collaboration with families, therapists and schools/services
Note: Where specialist behaviour support or restrictive practice authorisation is required, we coordinate with your authorised providers and follow all legal and NDIS requirements.

Community Access & Social Participation

Stay connected and active with support to get out and about. We help you explore interests, attend appointments and build social confidence.

Health & Wellbeing Monitoring

We keep an eye on daily health needs and coordinate with your healthcare team as required. Support workers follow clear instructions and escalate concerns appropriately.

  • Support to follow diet, exercise and therapy plans
  • Appointment reminders and assistance with bookings

Medication support and prompts

Medication Support (as per plan)

Medication prompts and assistance are provided only in line with your support plan and relevant policies. We maintain records and escalate any concerns.

  • Storage guidance and incident/variation reporting

Mobility & Transfers

Safe movement in and around the home and community. We follow manual handling plans and use the right equipment every time.

  • Support with walkers, wheelchairs, hoists and other aids
  • Guided practice to improve balance and confidence (as advised)
  • Vehicle transfers and safe transport assistance

Companionship & Carer Breaks

Friendly company and meaningful activity—plus short breaks for family carers when possible within your plan and roster.

  • Conversation, games, hobbies and interest-based activities
  • Short community outings, coffee and social catch-ups
  • Short-term in-home support to allow carer respite (as per funding)
Scope & Inclusions: We provide non-clinical disability supports aligned to your NDIS plan and goals. Exclusions: complex clinical nursing, heavy trades or major home modifications are referred to specialist providers; we’re happy to coordinate referrals.
